Adena Home Health makes house calls, bringing expert, skilled care to you. We know the challenges of treating a family member with a serious illness. That’s why our team of caring nurses, therapists, social workers and home health aids make sure that you or a loved one receive the care you need in the comfort of home. 

We provide skilled health care for adults of all ages in Fayette, Highland, Hocking, Jackson, Pickaway, Pike, Ross and Vinton counties. Our patient services are provided without bias or regard to disability (mental or physical), communicable disease or place of national origin.

Adena Home Health is a new joint venture partnership with national home health leader Alternate Solutions Health Network (ASHN), based in Kettering, Ohio, to advance home health and hospice services across south central and southern Ohio. A health care provider referral is required for these services.

Admission Criteria

To be considered for home care, you or your loved one:

  • Must be available for home visits and be willing and able to participate in your care
  • Have the need for skilled care services
  • Are under the direct care of a physician who will sign home health orders
  • Have difficulty leaving home without assistance
  • Have seen a health care provider in the last 90 days for a face-to-face visit and received a referral for home care
  • Have new or worsening symptoms 
  • Meet additional criteria based on your insurance carrier

Adena At Home

If you or a loved one are recovering from COVID-19, Adena at Home is here for you. Our patient navigators, home health nurses and remote patient telemonitoring services can help move recovering Coronavirus patients from the hospital to their home.

When you leave the hospital, we will provide:

  • A COVID-19 symptom tracker form
  • An iPad with video and messaging capability
  • Blood pressure cuff
  • Educational materials such isolate at home and manage your symptoms 
  • Pulse oximeter
  • Thermometer
  • Tools for monitoring symptoms such as a shortness of breath 

Using these tools, you can provide information that helps us monitor your condition. You will receive a daily phone or video status call from a registered nurse for 7-10 days to see how you are doing, as well as virtual rounds from Adena physicians and advanced practice providers. You also have access to 24/7 support from a Population Health or an Adena Home Health nurse if you have questions or concerns overnight.
